Job Description

Division: Coordinated Entry and Supportive Housing

Reports to: Coordinated Entry Supervisor

FLSA Class: Non-exempt

Job Type: Full Time

Salary: $20.50/hr (Grade 6, Step 1)

POSITION SUMMARY

Coordinated Entry Navigators meet with clients and conduct assessments to provide a wide variety of referrals and advocacy for homeless and low-income individuals. Identify long-term housing solutions, and support households to become permanently housed. The Navigator will also be responsible for documenting and entering appropriate information into the Homeless Information Management System (HMIS). They will establish and maintain effective partnerships with other area service providers and landlords.

DUTIES & RESPONSBILITIES

  • Meets with individuals seeking service to assess their immediate and/or long-term needs, including housing.

  • Identifies household's other primary barriers to housing (employment, substance abuse, mental health, education, domestic violence, and health) and coordinates resource referrals for these services.

  • Maintains an awareness of the criteria for various programs and screens individuals for program eligibility, and coordinates referrals as appropriate.

  • Maintains complete client files and ensures that accurate and complete information is entered into the appropriate data systems.

  • Assists the team in tracking, collecting and compiling statistics and in the completion of program reports.

  • Maintains a high degree of confidentiality regarding personal client information and manages information appropriately.

  • Proactively seeks awareness of current community resources and applies to clients appropriately.

  • Proactively seeks to recognize and understand the rules and processes required to successfully administer the coordinated entry program.

  • Participates in meeting, conferences, training sessions and workshops as assigned.

  • Initiate and process appropriate paperwork to secure payments for services according to contracts or grants.

  • Accepts other job-related duties as assigned.

REQUIRED QUALIFICATIONS

  • High School diploma or GED

  • Demonstrated successful experience working in direct client service

  • Some experience working digitally with MS365 Suite, including Word, Excel, PowerPoint, Outlook, Teams

  • Recognize the socio-economic problems and needs of homeless and low-income families and individuals

PREFERRED QUALIFICATIONS

  • AA degree or further education and/or experience in social services

  • Demonstrated successful experience in utilizing data management software

  • Lived experience in navigating the community resources

  • Familiar with available community resources

  • Multi-lingual

ESSENTIAL PHYSICAL/MENTAL FUNCTIONS

  • Requires walking, reaching/stretching, intermittent sitting, twisting and bending.

  • Must be able to occasionally lift up to 20 pounds.

  • Must be able to communicate with individuals, with a broad range of backgrounds and circumstances, in a positive and non-judgmental manner.

  • Must be able to successfully communicate both verbally and in writing in English.

  • Requires accuracy

  • The ability to discern courses of action to support clients

  • Successfully manage interpersonal contacts under a wide variety of conditions with fair and equitable treatment.

  • Must be able to exhibit regular attendance.
