• Qualifications

• High School Diploma or GED required

• Work Experience in the field of Developmental Disabilities/OPWDD with children or adults preferred

• Sensitivity to the needs of the individual

• Genuine interest in teaching, and willingness to motivate individuals to learn

• Valid New York State License, current insurance and liability coverage

• Strong command of verbal and written communication (English)

• Accountability

• Directly accountable to the individual primary caregivers , and indirectly accountable to the Program Coordinator Self Direction

• Specific Responsibilities

A. Directly responsible for the specific individual assignedB. Responsible for entering daily notes based on valued outcomes/goals worked on with the SD Individual D. Responsible for completing Monthly Summary Notes E. Complete assigned documentationF. Prepare for emergenciesG. Responsible for carrying out daily schedules as developed and plannedH. Ensures that individual is properly accounted for when transitioning to and from various locationsI. Implements weekly and daily programs to assist consumer in attaining his/herJ. Coordinates meaningful community integration opportunities for the specific IndividualK. Punching in and out via an EVV App on an Android or Apple device
