Guthrie Pharmacy Clinical Specialist - Infectious Diseases - Pharmacy - Full Time in Sayre, Pennsylvania

Position Summary:

Coordinates clinical activities in accordance with the Antimicrobial Stewardship program. Organizes, performs, and assigns the clinical activities of pharmacists and supportive personnel. Coordinates the clinical functions with daily work activities to maintain efficient departmental operation.

Education, License & Cert:

Graduation from an ACPE accredited School of Pharmacy with an advanced degree (Pharm D. or MS).

Currently licensed to practice Pharmacy in the Commonwealth of Pennsylvania and State of New York, or ability to acquire within 180 days of hire

Advanced training (residency, fellowship and/or board certification) in the area of infectious disease strongly preferred at hire

Experience:

Three years of experience in the area of infectious diseases or equivalent experience obtained through post-graduate training in the area of infectious disease

Competencies:

The clinical specialist must demonstrate growth and development of knowledge required to assess data reflective of the patients status, interpret the appropriate information needed to identify each patients requirements and to provide the pharmaceutical care needed, appropriate to the age of the patient and in accordance with appropriate policies and procedures. Reflective of these requirements, the clinical specialist must have completed ASHP Clinical Skills Program Module. Demonstrate competency in pharmacodynamics and pharmacokinetic monitoring. Demonstrate competency in monitoring drug regimens and making recommendations in the following areas: antibiotic utilization, therapeutic duplication, and unnecessary medication use.

The clinical specialist must be adept in documentation of clinical activities. The clinical specialist must actively participate in one of the professional pharmacy organizations. The clinical specialist must be able to demonstrate the knowledge and skills necessary to interpret and provide the required prospective drug review patient counseling. The clinical specialist must participate in relevant ACPE continuing education programs, with a minimum of 50% of the required hours obtained in a ‘live’ setting, in compliance with applicable state licensure requirements. The pharmacist must provide documented proof of C.E. credits earned as per mandated requirements clinical specialist must demonstrate age ‐ specific communication skills.

PATIENT AGES: The Employee pharmacist must demonstrate the ability to meet the pharmaceutical care needs of neonatal, pediatric, adolescent, adult, and geriatric patients.

PATIENT TYPES: The pharmacist must demonstrate the ability to meet the pharmaceutical care needs of acute care, intensive care, psychiatric care, sub ‐ acute care, long term care, oncology, and ambulatory/home infusion patients.

Essential Functions:

  1. Lead the development, implementation, and Chairmanship of the system-wide Antimicrobial Stewardship program

  2. Sets goals and develops plans to accomplish such goals to meet the requirements of the pharmacy clinical functions processes. Develops or participates in the development of policies, procedures, and guidelines for clinical pharmacy activities throughout the health-system

  3. Communicates with all staff members and provides the direct supervision and assistance necessary to ensure their compliance to meet requirements for clinical activities.

  4. Supports training initiatives for key stakeholders including all levels of healthcare providers and patients for ASP related clinical activities, documentation, tracking, reporting key metrics

  5. Ensures that appropriate personnel receive orientation, in ‐ service education and continuing education.

  6. Develops and ensures conformance to requirements for pharmacy based interdepartmental educational activities.

  7. Remains abreast of developments in the field through continuing education, review of professional literature and communication with peers and other organizations.

  8. Maintains records related to the cost impact of quality in the clinical activities quality improvement process.

  9. Analyze system data and build QI initiatives around antimicrobial stewardship using best practice methodologies

    1. Be key pharmacy contact for interdisciplinary system ASP partners (infection control, microbiology, infectious diseases, nursing, etc)

    2. Serve on system committees and workgroups as antimicrobial expert

    3. Provide support to rounding ID teams within Guthrie

Other Duties:

  1. Performs the activities of a staff pharmacist as required to ensure proper handling, preparation, and prompt dispensing of medications.
